Role Overview:

The Clinical Patient Care Coordinator holds a pivotal role in managing the care of medically fragile patients with complex needs, with a focus on Pediatric Critical Care. This role Includes ensuring high-quality patient care, with seamless care coordination, and acting as the central link between specialty providers and the clinical team. The responsibilities include actively engaging with families to address their concerns, supporting clinical staff through training and onboarding, delivering hands-on patient care as needed, and maintaining availability for on-call responsibilities during evenings, nights, weekends, and holidays.

Key Responsibilities:

  • Coordinate Care:  Oversee and coordinate the care of medically fragile patients with complex care needs, ensuring continuity and effectiveness of care delivery.
  • Transport Patients: Arranging and facilitating transportation for patients to specialty care provider appointments as necessary, ensuring prompt and secure transfers of patient.
  • Liaison Role: Act as a liaison between specialty care providers and the clinical team at the Pediatric Critical Care House, facilitating communication and collaboration to optimize patient outcomes.
  • Provide Updates: Regularly provide updates to the Clinical Director and Clinical Team on patient care management and progress, Incorporating any notable changes or developments in the patient's condition.
  • Case Management: Administer integrated case management services, ensuring continuous assessment, planning, implementation, coordination, monitoring, and evaluation of patient care plans.
  • Parent Relationship: Initiating and supporting a positive relationship with parents and families, by providing education, and guidance throughout the care process.
  • Leadership Responsibilities:  Monitor admission documentation, POT, POC, quality outcomes, and risk management processes, including addressing incident reports in collaboration with the Clinical team.
  • Reporting: Collect, compile and evaluate data on patient care and outcomes, producing detailed reports for the Clinical Director and organizational leadership as required.
  • Quality Outcome:  Work closely with the Clinical Department Leadership Team to evaluate patient progress, identify challenges, and recommend solutions and strategic plans to enhance patient care and achieve both departmental and organizational objectives.
  • Clinical Staff Training and Onboarding:  Assist the clinical staff training and onboarding processes, which includes providing mentorship and guidance when required.
  • Direct Patient Care: Provide hands-on patient care as required, upholding high standards of clinical quality and ensuring patient safety.
  • On-Call Availability: Be accessible for on-call duties during evenings, nights, weekends, and holidays as required to respond to emergent patient care needs and assist the Clinical Team.

The Profile

  • Registered Nurse (RN) license in good standing.  Current RN license in Washington State.
  • Bachelor's degree in nursing or related field (master’s degree preferred).
  • Minimum of 3 years of experience in pediatric critical care or a related clinical setting.
  • Should have strong experience in managing Trach-ventilator care for pediatric patients.
  • Should have good experience in NICU or ICU care.
  • Strong clinical assessment and care coordination skills.
  • Excellent communication, interpersonal, and organizational abilities.
  • Ability to work effectively in a fast-paced and dynamic healthcare environment.
  • Proficiency in electronic health record (EHR) systems and Microsoft Office Suite.
  • Commitment to maintaining confidentiality and upholding ethical standards in patient care.
